Pennsylvania Breast Cancer Coalition HIPAA Privacy Policy

We at the Pennsylvania Breast Cancer Coalition ("PBCC") remain dedicated to keeping the health information of you and your loved ones private. It has always been our policy, since our inception, to protect all of your personal information: we do not sell or share any of your individual data with anyone. Period.

Recently, a federal privacy law was enacted called the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). This law has created limitations on the use, disclosure and security of individually identifiable health information, or IIHI. While this law applies directly to doctors, hospitals, medical insurers and other covered healthcare entities (known as "covered entities"), the law has been expanded to include organizations which interact with, and serve congruent purposes of, covered entities (known as "business associates"). The PBCC has elected to follow these business associate guidelines in order to fulfill the spirit and the language of the law and to continue to exceed the expectations of those we serve.

Generally

The Health Insurance Portability and Accountability Act of 1996, also known as "HIPAA," has created new standards in privacy and security as related to individuals' health information. HIPAA generally requires that all individually identifiable health information be protected by covered entities and by their business associates. Covered entities include doctors, hospitals, emergency centers, health insurance companies, health care clearinghouses, and other forms of medical providers. Business Associates are entities that perform certain types of functions and transmit and/or receive individuals' health information.

Under the HIPAA Privacy Rule, this health information is known as protected health information, or PHI. PHI must be protected whether it is being used internally, within an office, or is being disclosed to other organizations. PHI must be tracked, logged and kept to a minimum of "use and disclosure," in compliance with the law.

The HIPAA Security Rule generally provides standards to protect the confidentiality, integrity and availability of your individual identifiable health information (IIHI). The Security Rule mandates internal safeguards for storage and maintenance, transmission and access to IIHI.

Our Commitment to You

The Pennsylvania Breast Cancer Coalition commits to the following:

  • We will not use or disclose your PHI, other than as permitted or as required by law;
  • We will use appropriate internal safeguards to prevent the use and disclosure of your PHI in any manner inconsistent with our stated purpose, or as required by applicable HIPAA regulations;
  • We will require our agents or subcontractors, who receive PHI from us, to follow protective standards equal to our own;
  • To the extent that we receive your PHI from a third party, we will keep track of that information, including when it is received and when it is disclosed; and
  • We will continue to maintain our standards at or above those required by state and federal law.

If you have any questions regarding HIPAA, please visit the U.S. Department of Health and Human Services website at http://www.hhs.gov/ocr/hipaa. If you have questions regarding our practices or your rights with respect to the Pennsylvania Breast Cancer Coalition, please contact our HIPAA Privacy Coordinator, Tricia Grove, at (717) 721-2580 x 105.

Very truly,

The Staff of the Pennsylvania Breast Cancer Coalition
