~ Reimbursement for documented travel expenses (not to exceed $100 total)

~ Be unable to attend the Conference without this grant

~ Live at least 50 miles from the Harrisburg Hilton

~ Must live or work in Pennsylvania

~ Complete and submit the grant application by August 3, 2012

~ Make your own travel and hotel arrangements

~ Check out of your hotel room by the posted time on October 9, 2012

~ Register by 8 a.m. with Kim at the Registration booth in the Harrisburg Hilton on October 9, 2012 with valid photo ID

~ Sign out and submit complete Conference evaluation form after the last workshop October 9, 2012

~ Mail completed travel reimbursement form and receipts by October 31, 2012 to PA Breast Cancer Coalition

~ Be unable to attend the conference without this scholarship

Approved grantees will receive approval letter and further instructions from the PBCC by Certified Mail by August 10, 2012. All other applicants will be notified by email or regular mail by August 10, 2012.
